Lateral Epicondylitis (Tennis Elbow)



Tennis elbow, a common musculoskeletal disorder also known as lateral epicondylitis, causes pain and tenderness on the outside of the elbow (Pathan & Sharath, 2023), which is frequently linked to repetitive motions, such as those in racquet sports. The term first came about in 1873 by a German researcher, Runge F. when he found that this condition occurs in regular tennis players where there is a clear association with the late back hand and forced wrist extension (Cutts et al, 2019). Thus, explains it as an overuse strain injury that occurs secondary to an eccentric overload of the common extensor tendon at the origin of the extensor carpi radialis brevis (ECRB) tendon (Buchanan & Varacallo, 2023). Currently the injury can also happen to individuals playing racquet sports with repetitive wrist motions other than tennis such as squash, badminton, etc.

Signs and symptoms
 


Pain and tenderness - pain outside the elbow (bony knob) with pain intensity ranging from mild discomfort to severe pain


 


Triggering factor -  pain aggravated with activities involving with or without loaded gripping and/or wrist extension movements such as racquet sports, lifting objects, or using hand tools

Grip strength - Gripping strength may be weakened, making some daily task to be difficult

Morning stiffness - in acute stages, stiffness and discomfort of the elbow can be felt, particularly in the morning, likely due to inflammation

Gradual onset - symptoms usually develop gradually over a period of time rather than suddenly

Conservative Management


Cold therapy - applying ice packs to affected area can help with reducing the inflammation and alleviate pain sensation (PRICE or RICE)

Ultrasound therapy - high frequency sounds waves penetrate the skin into the cellular level to increase cell activity, promote blood flow and accelerate healing


 

 

Manual therapy - Soft Tissue Mobilization such as MyoFascial Release can help relax the tensed and tight muscles and reduce pain

  • Joint Mobilization can help improve joint functions and reduce stiffness
 

Exercise therapy - Stretching exercises on the forearm muscles and tendons can help improve flexibility and reduce strain

  • Strengthening exercises (eccentrically in early stages) to help improve muscle endurance (gradually) and support for the affected tendons
  • Functional exercises to stimulate daily task or sports specific movements to ensure the affected arm can perform to pre-injury state.
 

 

Taping or Bracing - Kinesiology tapes or elbow braces can be use to provide support and alleviate further stress on the tendons
